Geriatric Care Manager

A reliable geriatric care manager will offer this range of service

Performs a client medical assessment, including physical and mental health.
Holds in-depth conversations with the elderly client about their situation and preferences, as well as (separately) with family members

Hold conferences with physicians

Learns and understands family support system and dynamics

Reviews finances, legal affairs, and insurance
Makes referrals to appropriate experts such as lawyers, accountants, physicians, and psycho-therapists
Explains Medicare, Medicaid, and other insurance programs

Coordinates medical care among doctors with different specialties

Searches out and recommends appropriate living facilities

Negotiates financial arrangements with living facilities

Develops an overall care plan
Lays out and recommends the best options based on the complete picture

Our Guava Care Team is professional and have gone through a very thorough interview process consisting of reference checks, criminal a background check, drug screening, Delaware Health and Social Services Adult Abuse Registry, training under the supervison of our Guava Team Leader. Daily check-in and check-out by phone is required of all caregivers along with weekly status calls or emails. We are very selective in our interview process and are committed to excellence in service.

We at Guava are communicators and ensure that your caregiver will be matched with your personality and level of care to accommodate all your needs.

If you need support for as little as 2 hours a day to 24 hours 7 days a week 365 days a year we have the solution! We can lead you through the process to ensure a smooth transistion working closely with families to ensure the highest quality of care with respect and dignity.

We require our Professional Care Team to have the following;

  • Delaware Health and Social Services Adult Abuse Registry report.
  • Health screening and proof of Hepatitis vaccination or delination
  • Criminal Background Check
  • Drug Screening
  • Reference Checks
  • Service Letters from prior employers
  • Professional apparel and issued ID badges
  • Review of all policy and procedure manuals prior to placement on the job
  • Current CPR certification
  • HIPP training
  • Orientation Training
  • On site employee checks and performace reviews. The reviews also consist of the input of the clients that we serve. We require our caregivers to document daily activities.
  • Documented progress reports are reviewed on a regular basis and ongoing phone communication with clients and staff.
  • Weekly progress reports to our out-of town family members to give updates on their loved ones either by phone or email.
  • On- call caregivers to address immediate schedule changes and emergencies. 24 hours a day 7 days a week 365 days a year.
  • Specialized traning and certificates.
  • Ensure all licenses and certifications are valid and in good standing.
